DO YOU NEED A ROOT CANAL?
A root canal becomes necessary when the soft tissue inside a tooth (the pulp) gets infected or inflamed. This can happen from a deep cavity, a crack in the tooth, a previous large filling, or trauma. Left untreated, the infection doesn’t resolve on its own. It spreads.
The most common signs that something is going on inside a tooth:
- Tooth pain that lingers after eating or drinking something hot or cold
- Pain when you bite down or put pressure on a specific tooth
- A toothache that wakes you up at night or throbs on its own
- Swelling in your gum near a tooth, or a small bump that comes and goes
- A tooth that has gone dark or discolored
- Jaw or ear pain that seems to radiate from one area of your mouth
You don’t need to have all of these symptoms. Some patients come in with obvious pain. Others come in for a routine exam and we find a problem on imaging before they’ve felt much at all. Either way, the sooner the infection is treated, the better the outcome.

Standard X-rays show a tooth in two dimensions. CBCT imaging shows it in three. That matters for finding hidden canals, hairline cracks, and the exact extent of infection before treatment begins.

MEET DR. OMAR GABR, ENDODONTIST
Dr. Omar Gabr, DDS, MS, is River Run’s in-house endodontist.
Dr. Gabr earned his Doctor of Dental Surgery degree with High Honors from the University of the Pacific, Arthur A. Dugoni School of Dentistry, and completed his specialty training in endodontics, receiving his Master of Science degree in the process. He is a member of the American Association of Endodontists, the American Dental Association, and the Minnesota Endodontic Association.
What that means practically: when you come to River Run for a root canal, you’re being seen by a doctor who has spent his career focused exclusively on this type of care, using technology he’s been trained on at a specialist level.
WHAT HAPPENS DURING A ROOT CANAL
The fear around root canals is almost always worse than the procedure itself. Most patients are surprised.
Here is what actually happens:
1. Exam and Imaging
Dr. Gabr reviews your X-rays and takes a CBCT scan to see the tooth in three dimensions. This step takes the guesswork out of what comes next.
2. Getting You Comfortable
The area around the tooth is fully numbed before anything else happens. We also have comfort options for anxious patients. You should not feel pain during the procedure.
3. Accessing the Pulp
A small opening is made in the crown of the tooth. Under magnification, Dr. Gabr locates and removes the infected pulp tissue.
4. Cleaning with GentleWave and Fotona
The canal system is cleaned using GentleWave’s acoustic fluid dynamics and Fotona laser irrigation. This combination reaches areas of the root that standard instruments cannot, reducing the chance of reinfection.
5. Sealing the Tooth
The clean canals are filled with gutta-percha, a biocompatible material that seals the space against bacteria. A temporary filling protects the tooth until your permanent restoration.
6. The Crown (Your Follow-up Visit)
Most teeth that have had root canal treatment need a crown to protect them long-term. Your River Run dentist handles this at a follow-up appointment, so you stay in the same practice for the full course of care.
ROOT CANAL VS. EXTRACTION: WHAT TO KNOW
When a tooth is badly infected, some patients wonder whether it makes more sense to just take it out. It’s a fair question and worth asking. Here is the honest answer.
Saving a natural tooth is almost always the better long-term outcome. Your natural tooth preserves the bone underneath it. When a tooth is removed, the jaw bone in that area begins to shrink over time. A gap also shifts the teeth on either side, which affects your bite and creates its own problems.
There are cases where extraction is the right call: when a tooth is too damaged to restore, or when the infection has progressed to a point where saving the tooth isn’t realistic. Dr. Gabr will tell you plainly which situation you’re in. We don’t recommend treatment just to recommend it.
AFTER YOUR ROOT CANAL
Most patients feel significantly better within a day or two. Some soreness in the area is normal for the first 48 to 72 hours and is typically manageable with over-the-counter pain relievers.
What to watch for: pain that gets worse instead of better after a few days, swelling that spreads, or a fever. If any of those happen, call us. We want to hear from you.
Once your tooth is fully healed, you’ll return to River Run for your permanent crown. After that, the tooth functions like any other tooth. Brush it, floss it, come in for your cleanings. Root canal-treated teeth can last a lifetime with normal care.
INSURANCE AND COST
Root canals are typically classified as a medical necessity by most insurance plans, which means they’re usually covered to some extent. The specifics depend on your plan: your deductible, your annual maximum, and whether the tooth being treated falls under basic or major services.

Does getting a root canal hurt?

Will I need a dental crown when getting a root canal?
Not always. That’s why it’s important to do routine checkups and to see us as soon as you start to feel pain. We may be able to catch the problem soon enough. However, you may need a dental crown if the tooth is too damaged from a cavity. Your dentist will let you know.

A root canal is a medical necessity under most insurance plans. So, yes, they are covered, but it’s best to check with your provider since other factors may apply such as waiting periods, premiums, and the like.
